HOUSING PROGRAM ASSISTANT - G Street - Posted 7/2/08 Part-time position in Transitional Housing program ~12 hours/week.

The Housing Program Assistant is responsible for supervising program participants who reside in Options' alcohol-free and drug-free recovery home, the Freedom House. This position requires familiarity with both addiction and mental health recovery issues.

The Housing Program Assistant works closely with a variety of community partners including, but not limited to: Josephine County Community Corrections, UCAN, DHS and others.

This position provides referrals, direction, and support to program participants and works collaboratively with a number of Options' social service programs. The Housing Program Assistant also provides ongoing analysis of program rules, policies and procedures, makes appropriate recommendations when necessary and keeps track of all program financials.

Candidate must be able to handle shifting priorities and frequent distractions. Physical Demands: The employee is regularly required to travel between sites in Josephine County and may be required to drive. Requires sufficient balance and mobility to participate in ProACT (Professional Assault Crisis Training) training and to implement its principles.

Hazardous Conditions: Risk of verbal and physical threats from clients.
Working Conditions: 50% office based, 50% field.
Mental health experience preferred; working knowledge of Microsoft Office a plus.
Base pay $10.00 per hour. No insurance benefits are available since this position is less than half-time.

Mental Health Technician or Mental Health Associate (QMHA) - Carnahan Court - Night Shift - Posted 7/2/08 Full-time position in a specialized residential treatment program for adults with psychiatric disabilities. Responsibilities include teaching life skills, managing symptoms, accessing community resources, providing clinical documentation and assisting residents in reaching their full potential. Also participates in facility cooking, cleaning and general maintenance duties.

This position is currently scheduled for three 12-hour awake overnight shifts each week from 6 PM to 6 AM, as well as 4 daytime hours to be arranged. Minimum one year relevant experience required. Three (3) years of relevant training, education and experience preferred; QMHA-eligible or BA in a human service field a plus - and may qualify for higher pay rate. Some computer skills required.

This full-time Case Manager provides recovery services to adults with severe and persistent mental illness.   Case managers follow evidence-based practices using the Strengths-Based model of case management, Assertive Community Treatment (ACT), and other program-specific evidence-based practices.  Services are primarily delivered in the clients’ natural environment (home, place of employment, neighborhood) rather than in an office setting.  Case managers help formulate and carry out individual treatment plans, which may include assistance with employment, housing, accessing health care, etc.

Candidate must be able to work independently and flexibly, under general supervision.  

One position is open, but it may be filled at either the Bachelor’s or Master’s degree level.

Case Manager - Intensive requires a Bachelor’s Degree in a behavioral science field – or equivalent combination of education and experience - as well as experience sufficient for designation as a Qualified Mental Health Associate (OAR 309-016-0005(58)).  This means that the Case Manager has the necessary experience and competencies to:

  1. Communicate effectively
  2. Understand mental health assessment, treatment and service terminology, and to apply the concepts; and
  3. Provide psychosocial skills development, and to implement interventions prescribed on a treatment plan within their scope of practice.

Registered Nurse for the ACT model Intensive Case Management Team - Posted 5/20/08

This nurse works as part of the Assertive Community Treatment (ACT) Team 32-40 hours per week.   The ACT Nurse facilitates mental health rehabilitation interventions and necessary supports to assist individuals with mental illnesses to achieve their rehabilitative, resilience and recovery goals. 

Responsibilities:
Supports our clients in their recovery through individualized support, skill training and case management.

Instructs individuals and families in health education and disease prevention and coordinates care to patients utilizing the nursing processes of assessment, planning, intervention, implementation and evaluation.

Visits clients in their homes to determine client and family needs, develops plans to meet needs and provides nursing services.

Works closely with consulting psychiatrist and medical doctors as assigned.  This may include medication administration and training, appointment scheduling and coordination of care as part of the client’s treatment plan.  Ensures that health care information is distributed to appropriate staff, psychiatrist, and other medical personnel as necessary.

Advocates with collateral sources to assist clients to receive the services and support they need to work on their recovery and independence goals.

Requirements:
The ACT nurse must effectively interact with clients, significant others, and other health team members.  S/he will always respect the clients’ right to determine the course of their lives.  This nurse will be computer literate, well organized, and able to fulfill all agency documentation and revenue goals.

Position requires a valid Oregon nursing license and a Bachelor’s degree in nursing, as well as experience sufficient for designation as a Qualified Mental Health Professional (OAR 309-016-0005-59).  This means that the nurse has the necessary competencies to:

  1. Identify precipitating events.
  2. Gather histories of mental and physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts.
  3. Assess family, social and work relationships.
  4. Conduct a mental status examination.
  5. Document a multiaxial DSM diagnosis.
  6. Write and supervise a treatment plan.
  7. Conduct a Comprehensive Mental Health Assessment.
  8. Provide individual, family and/or group therapy within the scope of their training.
